PARTICIPANT WAIVER, RELEASE OF LIABILITY & PAYMENT AGREEMENT

I, the undersigned participant (or parent/legal guardian, if the participant is a minor), voluntarily choose to participate in Pilates classes, training sessions, and other activities offered or sponsored by Pulse Pilates LLC ("Pulse Pilates").

ASSUMPTION OF RISK

I understand that participation in Pilates and other physical fitness activities involves inherent risks, including, but not limited to, muscle strains, sprains, falls, physical injury, aggravation of pre-existing conditions, and other injuries that may occur during physical activity.

RELEASE AND WAIVER OF LIABILITY

To the fullest extent permitted by applicable law, I agree to release and hold harmless Pulse Pilates LLC, including its owners, instructors, employees, contractors, organizers, sponsors, representatives, and agents, from claims arising out of or relating to my participation in Pulse Pilates activities, except to the extent that liability cannot legally be waived or released.

I understand that this agreement is intended to serve as a release and waiver of liability to the fullest extent permitted by law.

PAYMENTS, MEMBERSHIPS & REFUNDS

I understand and agree that, except where otherwise required by law or expressly stated in writing by Pulse Pilates LLC, payments and fees are non-refundable.

If I choose to discontinue participation after registering or entering into a membership or payment agreement, understand that discontinuing attendance does not automatically cancel any outstanding financial obligation under that agreement.

I agree to make all required payments according to the payment schedule and terms of my applicable membership, package, or contract until my financial obligations have been fulfilled.

CLASS REGISTRATION, CANCELLATIONS & MISSED CLASSES

I understand that I am responsible for registering for classes through the class sign-up system or links provided by Pulse Pilates LLC.

I acknowledge that I have reviewed and understand the applicable class cancellation and missed-class policies. including any late-cancellation, no-show, or other fees described in those policies. I agree to comply with these policies.

ACKNOWLEDGMENT OF TERMS

I confirm that I have read and understand this Participant Waiver, Release of Liability & Payment Agreement, as well as the applicable Pulse Pilates LLC membership, payment, cancellation, and class policies provided to me.

I understand that by signing below, I am agreeing to these terms voluntarily and that this document may affect my legal rights.
